1. Decision ownership

Name the single role accountable for automated change approvals in ServiceNow.
Name the role accountable for AI-influenced incident prioritisation.
Name the role accountable for automated security containment actions.

All three roles are named and documented.

2. Decision boundaries

Define what the system can recommend but not execute without human confirmation.
Define what it can execute automatically within fixed parameters.
Define thresholds that always trigger mandatory human review (regardless of risk score).

Boundaries are written down and enforced in the platform.

3. Escalation

When risk thresholds are crossed, does ServiceNow notify decision owners automatically?
Is escalation built into the workflow (no reliance on someone checking a dashboard)?

Escalation is platform-native and automatic.

4. Audit trail

Can you reconstruct what data was evaluated, what the system recommended, and who approved or overrode it?
Could you produce a decision audit trail within 24 hours if an automated action caused an outage?

High-impact decisions leave a reviewable trail.

5. Governance review

In your last governance review, were AI-driven decisions explicitly in scope (not only access and deployment)?
